General Waiver:

Heart and Soul Dog Breeding, Inc. (HSDB) will endeavor to create as safe an environment as possible for the training and care of the Dog (s) worked with (from now on, the "Dog") and will offer only sound safe, and responsible training, care, and post-training instructions. All animals may exhibit unpredictable behaviors, and HSDB will not be liable for the actions of the Dog that are outside the control of HSDB. The Volunteer will follow all instructions for care, handling, and equipment and will not leave equipment unattended or overnight on the Dog. A volunteer is responsible for maintaining control of the Dog and for the successful training, management, and conduct at all times. This Service Agreement covers the relationship between the Volunteer and HSDB each time the Volunteer works with HSDB. Volunteer affirms the terms of this Service Agreement and the truthfulness and accuracy of all statements Volunteer has made regarding Volunteer and Dog/s. You (the Dog Volunteer) acknowledge and understand that certain risks involve dog training, boarding, and care. You, at this moment, release and agree to hold HSDB, its Volunteers, employees, family, guests, visitors, associates, and agents harmless from any manner of injuries or death, damages, claims, losses, liabilities, costs, or expenses, causes of action or suits, whatsoever in law or equity (including, without limitation, attorney's fees and related expenses) arising out of or related to any services provided by HSDB, its Volunteers, employees, associates or agents, even under terms and situations of gross neglect. HSDB will not provide work with HSDB without your complete agreement with the Service Agreement. Your signature conveys you understand and agree to all content and conditions specified within the Service Agreement. Terms and conditions of this Service Agreement may not be changed or updated. By entering into this agreement, you agree to indemnify, defend, protect, and hold HSDB, its Volunteers, employees, associates, and agents harmless from and against any damages, claims, losses, costs, liabilities, or expenses (including, but not limited to, injuries to any person/animal or destruction of any property) arising out of your Dog's behavior, regardless of whether or not your Dog has exhibited such behavior in the past or the actions or inactions of HSDB, its Volunteers, employees, associates or agents. Photo/Video Release I, at this moment, assign and grant HSDB the right and permission to use, reproduce, distribute, and publish the photograph(s), film(s), videotape(s), audio and video recording(s), electronic representation(s) and sound recording(s) made of me and my Dog (s) at any time during the training and boarding of my Dog (s) by HSDB, and I at this moment release HSDB from any liability resulting from such use, reproduction, distribution, and publication. I, at this moment, authorize the reproduction, sale, distribution, copyright, exhibit, broadcast, and electronic storage of any such photograph(s), film(s), videotape(s), audio and video recording(s), electronic representation(s) and sound recording(s) without limitation at the discretion of HSDB. I expressly waive any right to any compensation I may have for any of the preceding. I authorize HSDB to run a background check, including the credit report, military history, etc. Upon receipt of any resignation or Board action to let someone go, they will be removed from the private Facebook groups and any other information sources not open to the public.
